Private Foster Care Agreement between Pet Owner
and Foster Pet Caregiver
This agreement
is made this _______ day of ______, 20__, by and
between
_____________________________(Foster pet caregiver) and ________________
(Pet Owner).
The pet is:

1. Foster
caregiver understands that he or she is not becoming the owner of any
animals, but is only willing to provide humane care for the pet until
its owner is able to fully care for the pet again or receive adequate
assistance to have pet in his or her home again.
2. Foster
caregiver agrees to peaceably and voluntarily give the pets back to the
owner at a convenient time for the foster home(within two weeks of the
contracted date) Foster caregiver agrees to make no claim of ownership,
title, right or interest in and to the animal.
3. Foster
caregiver agrees to let the Pet owner check the living quarters for the
pet at any time to ensure that the pet is being well-cared for. Foster
pet caregiver agrees to keep a collar and identification tag on the pet
at all times.
4. Foster
caregiver agrees to notify the Pet Owner as to any behavioural or health
problems of the animals. These concerns will be discussed with the
owner, who reserves the right to determine proper course of action to
take.
5. Foster
caregiver will notify the Pet Owner in the event of any change in
address or phone number listed below. Foster caregiver will not place
animal with anybody else unless it has been previously discussed and
approved by the Pet Owner
6 Pet owner
will provide supplies for daily pet care including food, medication,
bedding, toys, grooming, vet costs, and whatever other particular daily
needs the pet has. Pet owner agrees to reimburse foster caregiver for
further known expenses that include:
In addition,
the Pet Owner is willing to reimburse the foster caregiver for
additional unforeseen costs without prior approval up to __________ .
Every effort will be made to discuss needs with owner.
7. Foster
caregiver is undertaking these obligations with no claim, now or in the
future, to any type of compensation or reimbursement for caring of the
pet. Foster caregiver is caring for pet out of his or her own free will.
8. Foster
caregiver agrees to provide adequate food, water, shelter, and kind
treatment for animals at all times. Dogs will not be kept outside
unsupervised and cats will not go outside at all (unless on a leash and
approved by owner) Foster caregiver must adhere to all state and local
animal laws. Foster caregiver agrees to follow additional written
instructions by the Pet Owner
9. The
agreement is for the length of time in foster care is from
____________to _______________(estimated that the owner will be unable
to care for pet) When longer than 2 months, the plan will be
re-evaluated.
10. Foster
caregiver will have first option to keep the pet if the owner no longer
is able to and owner has no other plan in place for the pet.
11. Foster
caregiver agrees that accidental animal bites or other injuries to
humans and other animals do occur, and agrees to hold harmless and
indemnify, and protect the Pet Owner, from any claim or suit filed by
anyone as a result of such an incident. Likewise, foster caregiver is
not responsible for such incidents as well and will not be held liable.
In addition, the Pet Owner will not be responsible if animal should
damage or destroy property belong to Foster Caregiver, or shall transfer
any disease or internal or external parasite to other animals belonging
to foster caregiver.

This
agreement is the entire agreement of the parties, and there are no
oral promises or representations made in addition to this contract
and it may only be changed in writing both by the Pet Owner and the
Foster Caregiver.
